Implementation (Feb 08) of changes to CD registers. Removes “prescribed format” and requires specified headings for information including proof of identity requirements for Sch 2 CDs. Running balances and prescriber ID are not yet legal requirements. Reference SI 2007 SI 2154 reg 4 (9)
